When to use and not to use the Grants Management Document Submission Form

The Grants Management Document Submission Form is specifically used for grants, cooperative agreements, non-federal contracts, and other actions such as add-ons and extensions.

With more frequency, the Grants Management Document Submission Form has been used when the proposal or action requested is related to a federal contract – whether that funding is direct from the government or as a subcontract, thus routing the proposal/action to the incorrect team within the Office of Sponsored Programs.

If your action is related to a federal contract, please indicate this information via another means (post-it, memo), so that the correct team in the Office of Sponsored Programs will receive your request in a timely manner and provide a quicker turnaround time for you.
